Caring Hands Community Services is actively seeking a dedicated and compassionate Community Living Coordinator (CLC) to join our growing team. Position Overview
The Community Living Coordinator (CLC) plays a key role in overseeing home provider placements, ensuring service quality, and supporting individuals in achieving their personal goals. This position blends case management, compliance oversight, provider support, and strong relationship building with families and community partners.

We are looking for someone who is organized, dependable, person-centered, and committed to high-quality service delivery.

Key Responsibilities

Service Coordination

  • Oversee assigned home provider placements and ensure safe, supportive living environments

  • Conduct regular home visits, documentation reviews, and compliance checks

  • Monitor Individual Service Plans (ISPs) and ensure goals are implemented

  • Support individuals in accessing community activities, appointments, and resources
     

Provider Support

  • Recruit, onboard, and support contracted home care providers

  • Offer ongoing coaching, training, and problem-solving support

  • Ensure providers follow Caring Hands policies and DHHS/BDS regulations
     

Compliance & Documentation

  • Maintain accurate case notes, incident reports, and service documentation

  • Ensure compliance with Medicaid waiver requirements and agency standards

  • Participate in audits, quality assurance reviews, and internal compliance processes
     

Communication & Collaboration

  • Serve as the primary liaison for families, guardians, case managers, and external partners

  • Attend ISP meetings, team meetings, and care coordination sessions

  • Communicate updates and concerns to agency leadership
     

Qualifications

  • Associate’s or Bachelor’s degree in Human Services, Psychology, Social Work, or related field (preferred)

  • Experience supporting individuals with developmental disabilities or HCBS waiver services

  • Strong organizational, communication, and problem-solving skills

  • Ability to work independently and manage multiple cases

  • Valid driver’s license and reliable transportation

  • Ability to pass background checks and complete required trainings
     

Schedule & Work Environment

  • Flexible schedule with a combination of office work, home visits, and community-based responsibilities

  • Occasional evening or weekend availability based on individual or provider needs
