Community Support Services (CSS)
CSS assists with daily living skills such as personal care, meal prep, laundry, and home organization.
Support includes tasks like grocery shopping, medication reminders, budgeting, and routines for readiness, cooking, and cleaning.
Staff tailor skill-building to each person, using visual aids and prompts, and help with appointments and navigating the community.
Supports are adjusted as independence increases, promoting dignity, safety, and self-reliance so individuals can confidently manage more of their day.

Crisis Response Services
Crisis Response provides rapid assistance in emergencies to stabilize and ensure safety. We create safety plans, coordinate with families, and work with community partners. Services include de-escalation, stabilization, and urgent resource connections. Staff use trauma-informed, person-centered approaches and prioritize clear communication. Follow-up involves coping skill coaching and referrals. We assess triggers and protective factors to lower future risk, and documentation supports ongoing care. Our aim is fast stabilization, durable safety plans, and stronger support networks.

Non‑Medical Transportation
We provide reliable transportation for appointments, work, school, and social activities, scheduling rides to match individual routines and program goals. This includes commuting to interviews, classes, and events. Drivers offer door-to-door assistance and follow safe route planning. Vehicles meet safety standards, with accommodations available. We coordinate pick-ups with caregivers, employers, and providers. Travel coaching teaches schedule reading and trip preparation, promoting punctuality and navigation skills. Transportation aligns with CPS/CSS plans for consistency, resulting in better access, engagement, and independence.

Personal Emergency Response Services (PERS)
PERS provides 24/7 monitoring and rapid response for added security. We help choose and set up devices like wearables, home units, and mobile apps, featuring fall alerts, one-touch help, and location sharing. Training shows how and when to use each device. Equipment is tested regularly, and contact lists kept up-to-date. Care plans connect PERS with home safety and community activities, while we coordinate with responders and family to ensure clear communication. Devices are reviewed annually to address changing needs. PERS supports in-person care by enabling faster assistance, lowering risk, and boosting confidence at home and in the community.

Community Integration Services (CIS)
CIS promotes community involvement by coaching conversation, self-advocacy, and peer skills. Activities include joining leagues, volunteering, and attending events. Staff support introductions and practice social interactions in real environments. We find inclusive and accessible opportunities, set individual integration goals, and track progress through new contacts, memberships, and leadership roles. The aim is broader networks, a sense of belonging, and lasting community engagement.

Residential Rehabilitation
Residential Rehabilitation provides supportive environments to build daily living, health, and social skills.
A) Group Homes
Our group homes are handicap accessible and adapted for hearing and visually impaired individuals.
Features include ramps, wide doorways, visual fire alerts, vibrating alarms, and clear wayfinding.
Examples: shared lifeskills sessions, meal planning, and community outings.
Staff offer 24/7 support, personalized goals, and peerbased learning.

B) Residential Shared Living
Our shared living residential services offer enhanced family care designed to support individuals with intellectual disabilities and acquired brain injuries in achieving stability, well-being, and fulfillment within their communities. Each service is meticulously tailored to address the distinct needs of every individual we serve.
We recruit community members who welcome individuals with disabilities into their homes, integrating them seamlessly into their family environments.
Our family care providers include single individuals, couples, and families with children, all of whom offer housing and residential services to those who might not otherwise have a home.
Every home care provider operates as a contracted professional, receiving comprehensive, around-the-clock support from a dedicated team of program managers, nurses, and clinicians employed by Caring Hand Community-Based Services.
Home care providers go beyond simply offering shelter; they assist residents in all aspects of daily living, including meal preparation, laundry, medication management, and coordination of medical care and appointments, among other responsibilities.
