Position Summary
Caring Hands Community Services of NH is seeking compassionate, reliable Home Care Providers to offer a safe, supportive, and person-centered home environment for adults with developmental disabilities, autism, or complex support needs. Providers deliver daily living assistance, community engagement, and emotional support while maintaining a nurturing household aligned with NH DHHS and HCBS waiver standards.

This role is ideal for individuals or families who want to make a meaningful difference by welcoming someone into their home and supporting their independence and quality of life.
 

Key Responsibilities
 

Daily Living & Personal Support

  • Assist with daily living skills such as bathing, grooming, dressing, hygiene, and meal preparation.

  • Support household routines, structure, and independence-building activities.

  • Provide medication reminders or MAP-compliant administration (if applicable).

  • Maintain a clean, safe, and organized home environment.
     

Community Integration

  • Support participation in community activities, appointments, and social opportunities.

  • Provide transportation as needed (mileage reimbursement may apply).

  • Encourage communication, skill-building, and meaningful engagement.
     

Health, Safety & Compliance

  • Ensure the individual’s physical and emotional safety at all times.

  • Follow Caring Hands and NH DHHS policies, including incident reporting and documentation.

  • Participate in ISP meetings, team communication, and required training.

  • Maintain compliance with background checks, home inspections, and certification standards.
     

Documentation & Communication

  • Complete progress notes, logs, and required reports accurately and on time.

  • Communicate effectively with service coordinators, guardians, and Caring Hands leadership.

  • Report changes in behavior, health, or home environment promptly.
     

⭐ Qualifications

  • Must be at least 21 years old.

  • High school diploma or equivalent required.

  • Valid driver’s license, reliable vehicle, and proof of insurance.

  • Ability to pass NH background checks, fingerprinting, and home inspection.

  • Experience supporting individuals with developmental disabilities preferred but not required — training provided.

  • Compassion, patience, reliability, and strong interpersonal skills.

  • Ability to maintain professional boundaries and follow structured care plans.
     

Home Requirements (for Shared Living / Home Provider Model)

  • A spare furnished bedroom for the individual.

  • Safe, clean, and stable home environment meeting NH certification standards.

  • Willingness to integrate the individual into daily household life.
     

Compensation

  • Competitive tax-free stipend, based on the individual’s support needs.

  • Additional reimbursement for respite, transportation, and specialized supports when applicable.
