Position Summary
Caring Hands Community Services of NH is seeking a compassionate and skilled Nurse (RN or LPN) to support adults with developmental disabilities in residential and community-based settings. The nurse provides clinical oversight, medication management support, health assessments, and training to staff and home providers to ensure safe, person-centered care aligned with NH DHHS and HCBS waiver standards.
This role is ideal for a nurse who values autonomy, teamwork, and meaningful work that directly improves the lives of vulnerable adults.
Key Responsibilities
Clinical Oversight & Health Management

Conduct routine and as-needed health assessments for individuals served.

Review medical histories, physician orders, and care plans to ensure accuracy and compliance.

Monitor chronic health conditions and collaborate with healthcare providers as needed.

Support care teams in recognizing and responding to changes in health status.
Medication Management

Oversee medication administration systems in accordance with NH Medication Administration Program (MAP) standards.

Review medication logs, MARs, and storage procedures for accuracy and compliance.

Provide MAP training, refreshers, and competency evaluations for staff and home providers.

Coordinate with pharmacies and providers regarding medication changes.
Training & Support

Train Direct Support Professionals (DSPs) and Home Providers on health-related protocols, documentation, and emergency procedures.

Provide ongoing coaching to ensure safe, consistent implementation of care plans.

Participate in ISP meetings and interdisciplinary team discussions.
Documentation & Compliance

Maintain accurate, timely clinical documentation in accordance with agency and state requirements.

Complete incident report reviews and follow-up actions.

Ensure compliance with NH DHHS, licensing, and waiver program standards.

Assist with audits, quality assurance reviews, and regulatory reporting.
Collaboration & Communication

Communicate effectively with families, guardians, case managers, and healthcare providers.

Provide clear, supportive guidance to staff and leadership regarding medical needs.

Participate in team meetings and contribute to person-centered planning.
Qualifications

Active NH RN or LPN license in good standing.

Experience working with individuals with developmental disabilities preferred.

Strong assessment, documentation, and communication skills.

Ability to work independently and manage multiple priorities.

Valid driver’s license and reliable transportation.

Ability to pass NH background checks and meet agency compliance requirements.
